71 year old male presented to the ED complaining of left leg pain. POCUS revealed free floating DVT in common femoral vein extending down to the popliteal vein. Free floating DVTs, especially those greater than 5cm, can increase a patient’s risk of developing a pulmonary embolism. Common femoral vein in transverse (left) and sagittal (right) views demonstrate a hyperechoic thrombus that is noncompressible with surrounding anechoic fluid suggesting free floating thrombus with circumferential blood flow.
