A 70 year old female patient with complaints of acute dyspnea. Oxygen saturation 86% with room-air improved with O2-suppletion. Poor hemodynamics with systolic presure around 100 mmHg. POCUS shows obvious right heart strain with apical sparing (McConnel's sign) and trombus drifting around in right atrium and even hitting the right ventricle. Correct therapy (trombolytics) was started almost straight away after presentation at the ED because POCUS was available. Dr. Van Roosmalen
